Obituary

Deacon Arthur "Chuck" Forman, Sr., 94, native and lifelong resident of Crescent City, entered the sunset of life on Monday, April 20, 2020 at his Smiley Church Road residence. The son of Charlie and Rosa (Hirst) Forman, (the seventh child of eleven children) "Chuck" was born on Wednesday, November 25, 1925. He was educated in the Putnam County School District, attending Denver Elementary School and Middleton High School, subsequently earning his general equivalency diploma in 1955. On March 19, 1946, Chuck united in marriage to the former Emma Jean Powell and they became the proud parents of three daughters, Joann, Rosa, and Sharon Rene'; and two sons, Arthur, Jr., and Richard "Pop." After a long illness at the Lantana Tuberculosis Sanatorium in Lake Worth, FL, he became interested in becoming a barber. During this time, he was going through recovery and rehabilitation when he was encouraged by his mentor, Addison Carey, to attend the Jones Institute in Jacksonville, FL. In 1957, he became a licensed professional barber and opened his own shop and became renowned all over town as "Chuck the Barber." No challenge was too great for Arthur "Chuck". Also in 1957, he applied for a custodian position and became a school custodian at Middleton Junior High School within the Putnam County School District and was faithful to his duties for over thirty years. He has served numerous generations of South Putnam County families as both barber and custodian. In 1958, Arthur and Emma Jean became proud owners of Forman's Fernery and Greens and built up over three-and-a–half acres of leatherleaf and tree fern. Through his many jobs, Arthur "Chuck" was very industrious and a dynamic provider for his family.

Chuck's involvement didn't conclude with his entrepreneurship, he was active in the community as well. He as a member of the Crescent City Auxiliary Police Program. He instituted the first African-American Boy Scout unit in the Crescent City region. He was a former brother in the Masonic order. In 1966, he was ordained as a deacon and devotedly served the Lord through Friendship Baptist Church, Crescent City. "Chuck truly loved God, his church and his community. He considered his biggest accomplishments to be, knowing God and encouraging his children and other young people to "Seek Ye, First the Kingdom of God." His life example teaches us to never give up when faced with illness or life challenges.

"Chuck" and Emma Jean were devoted to God and one another for 69 years until she preceded him in eternal rest on January 26, 2016. In addition to his parents and wife, he was preceded in eternal rest by his daughter, Joann Forman Morgan; son, Richard "Pop" Forman; siblings, Charles, Carrie, Jake, Wilder, Nellie, Annie Mae, Louise, Alice, Isaac, and Henry.

To cherish his memories: daughter, Rosa (Paul) Hudson, Savannah, GA; son, Arthur Forman, Jr., St. Augustine; daughter, Sharon Rene' (Rodney) Raines, Alpharetta, GA; three grandchildren, Terrance (Cornethia) Forman, Palatka; Lauren Elliott, New Haven, CT; Rachel Raines, Atlanta, GA; one great grandson, Terrance Forman, Jr., Palatka; devoted brother-in-law, Elder Theodore (Ruby) Powell, Crestview, FL; and a host of nieces, nephews, other relatives, god-children and friends.

Due to the current COVID-19 pandemic and the Governor's executive stay-at-home order, the Forman family will conduct a private Homegoing Celebration. A limited, socially-distanced visitation will be held from 4-5 p.m., on Saturday, April 25, at Friendship Baptist Church, 286 Smiley Store Road, Crescent City, Rev. Michael Robinson, pastor, Officiating. Private interment will be in Evergreen Cemetery, Crescent City.
